PART III Individualized Care Planning: Getting to Know the Person (1 hr 19 mins)
Regardless of your setting, this presentation will help you find new ways to get to know the elders you serve and better meet their needs. Discover ways to put each person back in the “driver’s seat of their lives” and how to soften the institutional assessment process. Review the federal assessment and care planning requirements as well as what goes into determining the “highest practicable level of well-being.”
